How to embed a Score Field into the customer portal

Since August 1, 2025, Data Forms for Jira includes a built-in Score field.

You can still create additional custom fields if needed: Create Custom Field with our Data Forms Score Field Type

You can use Score fields not only for work items (issues) but also in the customer portal.

Below are the steps for using the built-in Score field:

  1. Open Work item view and drag the Score field into one of the field sections.

  2. Open Request form and drag the Score field into the request form.

When a customer fills out the form, the Score field will be displayed for them to complete.

After submission, the calculated score will be shown to the customer in their request.

Agents will also see the score in the request.

Additionally, agents can edit the Score field if needed.
