When trying to add a field to the IssueForms create form macro for a certain project, the select list is empty.

Solution

In order to add fields to the Create Form Macro you need Create Issue permission for that particular Jira project.

Here are two ways to fix this:

1. The Project Administrator grants you the Create Issue Permission by assigning you a project role with these rights.

2. The Jira Administrator changes the permission scheme of that project so that your role or a group that you are in has Create Issue Permission.
