Import the following template (Data Forms templates - How to export and import templates):

Create Upgrade Checklist

  1. Open a Jira issue and click Control button within the Data Forms section

  2. Click on the Create button at the “Zero downtime Upgrade Checklist Jira“ template

 

Add Upgrade Checklist

  1. You can specify a Data Form Id for the checklist

  2. In the text field you can check activities that have already been performed

  3. Press the Save button

 

 

As you can see, the checklist attached to the Jira issue.
