How to add a data form to a Confluence page

In order to add a data form to a Confluence page a template must first be created. Data Forms for Confluence comes with 3 predefined form templates to chose from. To add your own templates please ask your Confluence administrator for help or check out our documentation on Data Forms fields - How to create, customise and delete Data Form fields and Data Forms templates - How to create, customise and delete Data Form templates here.

  1. On your Confluence page click on the Data Forms Control button. (Note that this will only be shown if there is no data form attached to this page. If there is a data form attached to this page the text of the button will be 2 data forms. The number indicating how many forms are already attached to this page.)

  2. The Data Forms Control panel will open. The panel shows you a list with the forms that are already attached to this page (if any) and a list with the predefined Data Forms templates you can chose from. To add a data form to the page click on the Create button behind the name of the template you want to add.

  3. The modal will stay open and show you the form you chose. You can enter values into the input fields if you want to.

  4. Click on Save to save the form to your page.

  5. Your newly added data form is now in the list of the attached forms.

If you want to display the newly added form on the page you can do so with the Data Forms macros. For further information on Data Forms View macro - How to display a data form on the page please check out our documentation.
